BELLAVITA CLUB is a VIP loyalty program providing rewards for our loyal customers. Please review the terms and conditions herein carefully before submitting this application. Your submission will be deemed as your acceptance to the terms and conditions. The terms and conditions are amendable by BELLAVITA and will be announced in BELLAVITA service center. If you are under age of 20, please submit the application with permission of your parents or guardians. Membership and benefits are designated for the card holders only and not transferable.
Anyone can register for temporary member of BELLAVITA CLUB with a qualified purchase receipt in BELLAVITA. Customers spending over NT$150,000 within one year will be upgraded to the Pearl Card, which is renewable every year for over NT$150,000 annual spending. Customers spending over NT$ 1.5 million within one year will be upgraded to the Gold Card, which is renewable every year for over NT$ 1 million annual spending. BELLAVITA will send out a written notice whenever a member status change occurs.
- To accumulate your spending, the Members are suggested to present member cards when purchasing. The Members may provide name, phone number and last four digits of ID number to log into the account in each store or may also bring the purchase receipts to the B1 service center for accumulation. Reward points can only be credited to your account on the day of purchase.
- All qualified purchases occur at BELLAVITA are eligible for accumulation except for the related special ineligible transactions, including but not limited to, gift card / voucher purchase, change / currency exchange, parking fee, or any non BELLAVITA in-store merchandise / service purchase.
- When a refund or exchange is needed, please present your member card at the counter to correct the accumulation.
- BELLAVITA will provide account statements regularly. The Members may also check such information at the B1service center during the business hours (10 : 30 - 22 : 00 Daily)
- Should your member card lost or damaged, please contact the B1 service center immediately.
An additional issuance fee of NTD $200 will be charged.
- BELLAVITA reserves the right to adjust the membership application, to review, cancel or change the terms and
conditions herein. Any change or update will be notified in writing or announced in the service center.
- The Members shall provide accurate and true personal information. Any members who provide false information or fraudulently use other person’s identity shall bear their own legal responsibilities. The Members shall timely notify BELLAVITA of any change to personal information; otherwise, BELLAVITA will not be responsible for any loss due to mailing failure of membership reward or other written communication. The Members are free to request to check, review, or make a copy of or ask to stop collecting, processing, or using their personal information by BELLAVITA at any time. Pursuant to the laws, any check, review, or copy of the personal information may be charged at NT$100 per request.
